from The Cats’ Cradle and agree to the following conditions:

1. If, for any reason, at any time during the cat's life, I am unable or unwilling to care for it, I agree to contact The Cats’ Cradle first, and I will not give the cat to anyone else.

2. I will be patient and review the 3-3-3 Rule of Adoption.

3. I agree to ALWAYS provide the animal with proper food, shelter, and water, as well as yearly vet checkups, vaccinations, and additional medical care if the need arises.

4. I will promptly notify The Cats’ Cradle of any change of my address or telephone number.

5. I will notify The Cats’ Cradle immediately if my cat ever gets lost or stolen and will make every effort to recover the cat/kitten.

6. I will never allow this cat to be declawed, used for vivisection/experimentation, dog fighting, or the purposes of

   entertainment, or any other purpose except as a house pet and companion.

7. I am able to provide my valid government ID (driver’s license, state ID, passport) as required by the North Carolina Department of Agriculture/Animal Welfare Services.

I understand and agree that if I break this contract in any way or if a The Cats’ Cradle representative decides that I am no longer qualified to keep the animal, then the animal may be taken away from me by The Cats’ Cradle and that I may be held accountable for any violation of this agreement.

I restate my understanding of my obligation by initialing here that I understand that accidental animal bites or other injuries to humans and other animals do occur, and agree to Hold Harmless and Indemnify, and Protect The Cats’ Cradle, from any claim or suit filed by anyone because of such an incident. In addition, The Cats’ Cradle will not be responsible if the animal(s) should damage or destroy property belonging to me or others or shall transfer any disease or internal or external parasites to other animals belonging to me or other animals.
